I have been receiving numerous from your company or a client that you provide service to.
I am hereby requesting you or your client (Credit card service, Member Service) cease and desist any more phone call to me or my personal cell phone number (xxxxxxx3 on this day of our lord Monday May 12, 2014. As requested I am submitting the following phone numbers calling my phone. Please note that when I try to call the number back, the number is no longer in service.

If After the completion of the 30 day I receive any more call from companies as describe above, callerid4u will be held responsible to all chargers incurred to me by my cell phone provider at 10X the amount plus Legal fee’s not limited to Attorney Fees court and filing fees.
